NPI Number Lookup

The National Provider Identifier, or NPI, is the unique identification number issued to health care providers in the U.S. by the Centers for Medicare and Medicaid Services (CMS). Most healthcare providers, individuals and organizations alike, that operate in the United States are required to obtain and use a NPI number in the ordinary course of their business. The registry cited below, which will open in a new window, is published by CMS and includes parts of the NPI record that have public relevance, including the provider's name, specialty (taxonomy) and practice address.
